Free Discovery Call
We offer the opportunity for you to visit with one of our patient care coordinators to have your questions answered and to provide you with more information about our services
Initial Functional Medicine Visits
Your initial Functional Medicine visit will be by telephone or video conference with one of our practitioners to review your Health Journey and Medical History information. We will also request you complete an advanced assessment evaluation prior to this call. This will help us to more clearly define the best starting point in your care. After this call you will be given information about recommendations for testing along with diet & lifestyle recommendations.
Follow-up Functional Medicine Visits
After testing has been completed you will be scheduled for your initial Case Review visit either in person if you are a new patient to Willowbend or by video conferencing if a returning patient. This visit will be a review of all clinical findings along with a brief physical exam and discussion of your proposed treatment plan. All questions will be addressed prior to initiation of your plan to ensure your success.
